Student dependants.

For students who are currently insured under policy Q178 through your university and who require coverage for their dependent spouse and/or children, you will find links below for:

•  a summary of the coverage available for your dependents
  and  an application form

Please note:

•  this policy will not cover any medical condition or pregnancy which commenced prior to the start of the coverage
• the rate of $495 will apply if you have 1 dependent child who is currently a full time student. 
•  the rate of $990 will apply if you have a dependent spouse and no children AND do not require any maternity coverage on the plan.
• the family rate of $1,485 will apply if you have a spouse and/or dependent children to insure.   Also if you wish to insure a dependent spouse, including any future pregnancy, this rate will apply.  This rate will apply for a maximum of 4 dependents.

 NOTE: This policy will only cover maternity related expenses, subject to the terms and conditions of the policy, if “family” coverage is in force at least 10 months prior to the anticipated date of delivery.

On completion of the application, payment may be made by:

• credit card or CERTIFIED cheque, payable to Telfer International Consultants Inc.

If payment is made by credit card, your application may be sent to Telfer International Consultants Inc. by fax, email or regular mail.

If payment is made by CERTIFIED check, your application any payment must be sent to Telfer International Consultants Inc. by regular mail.

Your policy will be returned to you by email, unless you specify that you would prefer a copy by regular mail.
